Our vision is to support young people to achieve their aspirations and map pathways to independence.
We support young people aged between 16 and 25 by encouraging them to focus on their strengths and build positive support networks to achieve their aspirations, whether that is returning to education, finding meaningful work or making new friendships.
We provide supported living placements for young people aged 16 and over. Our main aim is to provide safe and high-quality accommodation in which our young people can develop and enhance their independent skills. Supported Living provides a nurturing, supportive environment for young people transitioning to independent adult living. We undertake a complete assessment for each young person to determine their abilities to provide an individual plan to develop independence, access further learning and to support young people to reach their goals and aspirations as they prepare for adult living.
We work in adherence with other relevant organisations to ensure that all young people are provided with a positive, consistent and excellent standard of care.
Our Supported Living service provides multiple occupancy accommodation for young people who are able to live independently with support from our staff. Our Supported Living service provides accommodation in a family-style home alongside other young people, all ready to live independently with support.
Our Supported Living service includes:
- Accommodation which is maintained, decorated and well furnished to a high standard.
- A tailored independent living programme.
- Support with future housing
- AQA registration and support
- Financial advice and assistance
- Access to advocacy services
- Support to register with local health providers
- Support in accessing education, employment and training
- Access to counselling and support in areas such as drug and alcohol, sexuality, sexual health, anger management, self- harm, safeguarding etc.
- Accommodation is available 365 days a year with 24-hour support
- Bespoke care packages and support hours dependent on individual needs
- Support young people experiencing family or relationship breakdown
- Support in joining the Care Leavers Association and a national voice for peer support and guidance.
Clearcare Healthcare employ a team of Project Workers who support the young people in our Supported Living homes across the counties. Our Project workers support young people to develop the practical and emotional skills required to live independently and reach their full potential.
We strongly believe that every young person has the right to be supported into a successful transition from childhood to adulthood, regardless of their background or circumstances.
